This is an appointment and a fee based only service. It is not intended to replace your local GP service or Emergency service that you currently have in place but complement the process of healing and recovery by functional integrative approach. To register in practice you need to have a doctors or practitioner consultation in person or exercise the fact you are the parent or guardian during the consultation.

Once the appointment is complete your blood samples will be delivered to the lab in Our Lady's of Lourdes hospital. The results will be sent directly to your GP or Consultant.

The certificate has two parts.
IB1 form - Ilness Benefit form and Injury Benefit - a booklet that is completed by the patient and, in the event of an accident at work, by the patient's employer. The IB1 form related to non-injury illness you may complete it online at mywelfare.ie
Certificate for incapacity for work - is completed by the doctor and then signed by the patient. In our practice, we only use paper certificates. Such a certificate should be sent to Social Welfare Services, P.O. Box 1650, Dublin 1. Due to the size of the practice and the population we care for, we are unable to send e-certificates.
